360° Self/Other Leadership
Assessment
Completing this assessment involves four steps.
First print out a hardcopy of each of the four forms listed below. Make copies
of the favor letter and distribute to the Others who will complete the Other
form. Complete the forms on the hardcopy -- you do the Self and the others do
the Other form -- before you submit them online. The Self form includes 43
questions about the Nine Behaviors of Leadership, which are numerical ranking.
Be sure to include examples in the narrative part. The Other form includes the
same 43 questions, only focused toward their perspective. When you and the
others have completed the hardcopy forms, you can more easily submit the
information online without disconnecting. After you have received the results of
the full 360, you should complete the Follow-up form which helps you develop a
specifc action plan.
